Glycoconjugate vaccines provide effective prophylaxis againstbacterial infections. To date, however, no commercial vaccinehas been available in which the key carbohydrate antigens areproduced synthetically. We describe the large-scale synthesis,pharmaceutical development, and clinical evaluation of a conjugatevaccine composed of a synthetic capsular polysaccharide antigenof Haemophilus influenzae type b (Hib). The vaccine was evaluatedin clinical trials in Cuba and showed long-term protective antibodytiters that compared favorably to licensed products preparedwith the Hib polysaccharide extracted from bacteria. This demonstratesthat access to synthetic complex carbohydratebased vaccinesis feasible and provides a basis for further development ofsimilar approaches for other human pathogens.
